Obesity is a chronic, relapsing, and multifactorial metabolic disease that remains a major contributor to global morbidity and mortality. The emergence of incretin-based therapies, particularly glucagon-like peptide-1 (GLP-1) receptor agonists and dual GLP-1/GIP agonists, has significantly advanced obesity treatment, demonstrating substantial weight loss, improved glycemic control, and favorable cardiometabolic outcomes. However, long-term success remains limited by weight regain, lean mass loss, and incomplete metabolic restoration.

This presentation proposes an integrative, physiology-driven framework that combines GLP-1–based pharmacotherapy with hormone optimization and longevity-focused interventions to address the underlying drivers of metabolic dysfunction. Key endocrine pathways—including insulin signaling, hypothalamic-pituitary-adrenal (HPA) axis regulation, thyroid function, and sex hormone balance—are evaluated for their roles in energy homeostasis, adiposity, and metabolic resilience.

Adjunctive strategies are explored to enhance treatment sustainability, including preservation of lean muscle mass, optimization of mitochondrial function, and reduction of chronic low-grade inflammation.
